What does it contain?
The document starts with some introductory text you can use in an email when you send out the Data Protection and Privacy Consent. I use WriteUpp and I take the client’s consent using their ‘Consent’ function.
Following this suggested email text the main document includes the following headings:
- Who I am and what service I am providing
- Why I need your personal data
- How I will use your data within my practice. This includes a list of the electronic products I use and briefly what I use them for. You can edit these to reflect your practice but this will prompt you to consider tools used across all aspects of your organisation
- How you can modify the data I hold about you
- How you can access data I hold about you
- How you can retract consent to hold and process your records
- How you can raise a complaint, should you wish to do so. This includes details of your professional body and the legal jurisdiction you operate under, particularly important for non-UK residents
What format does the document come in?
- Microsoft Word Template: Editable and unbranded so you can add your own branding
- PDF Template* in Five Colourways (Teal, Orange, Charcoal, Lime, Navy): Editable and unbranded so you can add your own branding
- Online Psychology and Counselling PDF : Non-editable fully branded example
* Please note the PDF templates are partially editable. You can insert your logo and company name and fields are available for clients to insert their information. However, as they are PDFs certain formatting features are fixed. You can however fully edit the Word documents which contain identical content.
