Consent to Share Information
This document gains the client’s informed consent for the therapist to share their data with named individuals based on the terms set out in GDPR

When is this completed?
This is administered as therapy unfolds and only if needed. I will already have gained consent regarding GP contact (see Client Details Form collected as standard), but if I need consent to share information with anyone else, I send this form out at that point.
What does it contain?
Headings are:
- Introductory paragraph situating this form in the context of GDPR.
- Principles of data sharing e.g. strictly need to know basis
- Duty of care and limits of confidentiality
- Who the client agrees to share their information with
- Who the client DOES NOT want information shared with
- Client’s name, date and signature
What format does the document come in?
- Microsoft Word Template: Editable and unbranded so you can add your own branding
